Not quite sure how well they would work on a "pitched" roof but I suspect it's readily possible with some modification to the upstands - they seemed pretty accomodating when I spoke to them at ECOBuild.

Seems much easier than making a kerb and fitting a velux style window, but probably more expensive to buy. Velux can do the flashings for you seperately, but you'd need to talk to their tech department and it will probably be expensive.
